2012 Vitality Awards Rules:

Professionals and/or volunteers working in the areas of direct healthcare, wellness and/or health education will be recognized in one of the following categories:
  • Nutrition & Wellness – Including direct care, community and academic education
  • Drug, Alcohol or Tobacco – Including prevention and/or treatment
  • Social & Emotional Well Being – Including mental health, bullying, counseling, or suicide prevention
  • Human Growth & Development – Including physical and dental hygiene, fitness, pregnancy prevention or education, STD/HIV prevention
  • Health Education – Including preschool, elementary, middle and high school teachers
Who is eligible: The Vitality Awards seek to recognize individuals in northeast Indiana and northwest Ohio region who are providing health care education, direct care and wellness programming to children and/or adults in our community.  You are encouraged to work closely with your nominee to complete the nomination. 
 
Criteria: Have a strong commitment and demonstrated effort to improve the quality of life in our community.
 
Nomination Process: 
Professional Nominees: Representing either the profit or not-for-profit sector, and working/volunteering in the Fort Wayne region. There is no minimum or maximum age qualification for the professional categories. Submissions should include 300-500 words about why the individual or organization should be recognized.
 
Student Nominees: Those pursuing a health/wellness career certificate or degree program, including juniors/seniors in high school, undergraduate or graduate students and returning adults are eligible to apply.  They must have attended at least one McMillen Center for Health Education program and in 300-500 words explain how this experience influenced their decision to pursue a health and/or wellness career. 
 
Deadline: Entries are due no later than March 16, 2012.  Submissions accepted online at www.mcmillencenter.org/events.  Mailed entries must be received no later than March 16, 2012. The McMillen Center is not responsible for nominations not received by the deadline.  Nominations will not be returned.  
 
Selection: All nominations will be reviewed by a selection committee comprised of industry professionals. This selection committee will determine all winners. The award recipient must be able to receive the Award in person at the Vitality Awards Celebration at Pine Valley Country Club on May 17, 2012 at 7 p.m.
 
Notification: Recipients will be contacted no later than March 31, 2012 of their award.  A biography and photo will be requested at that time for publishing in the Vitality Awards program.
